Patient Responsibilities:
1. Must be a current patient on our active roster, and should have a cleaning and exam prior to tray fitting appointment.
2. Must keep appointments for regular scheduled cleaning, exams, and necessary x-rays.
3. If a change comes up in your schedule, we require 48 hours notice so other patients can be served during your previously scheduled appointment. Also we do require that patients schedule within two weeks of previously scheduled appointment date.
4. Patient must agree that results vary between patients, and for some patients tray whitening may not be the treatment necessary for desired results, and that Grandon Village Dental office is not liable financially or in any other way for your whitening experience. The patient must agree that we are not responsible for improper application or use, and that if whitening gel is improperly applied their could be health issues as a result. Patient will assume all financial responsibility for any expenses associated with results from improper use or application of whitening trays and gels.
5. If a patient were to have any negative health reaction to whitening gels, stop usage immediately, and contact your physician or dentist right away.
6. Failure to follow any of the above patient responsibilities, may result in termination of the whitening for life program for that patient as we believe we have given every opportunity to properly participate and make necessary changes.
7. Grandon Village Dental Office ownership and management retain the right to review each patients program, and are able to stop program enrollment at anytime if deemed necessary. Also management agrees to take each case and look thoughtfully at any disputed details and holds final resolution authority.
8. If a patient is unable to remain in the whitening for life program, normal fees will apply for any future services associated with teeth whitening.
